16th Annual NCSSSMST Student Research Symposium

In order to participate in the Symposium, each student participant must review, sign and submit the following forms:

Participant Agreement (pdf)

Parent Permission (pdf)

Medical Release and Emergency Contacts (pdf)

All of the above forms should be signed, and mailed to the following address by May 7, 2009:
